Important: Please note that the specific application documents/information below are applicable to case study discussions and peer supervision only.
- The completed CPD 2A Form (the application form).
- Programme of the CPD activity specifying the exact time slots for each presentation/topic (please note that case study discussions and peer supervision applications may be awarded up to a maximum of 10 CPD points per annum).
- The planned advert for the CPD activity (if applicable).
- Presenter(s) short CV / biography.
- A summary of the presentations (per topics listed in the programme) or a copy of the full presentations in PDF/PPT format must be presented. Ethics presentations require very detailed summaries.
- The sample attendance register to be used to monitor attendance or the monitoring mechanism to be used if online activity.
- Evaluation form (for physical attendance activities) or MCQ’s (for online activities).

Important: Applications for case study discussions and peer supervision are classified under “small groups” and thus, charged per application and the total number of participants in small group activities may not exceed ten, per annum.
